Please arrive at least 15 minutes before your scheduled tryout session. A parent or guardian must accompany all players.

Late arrivals will be admitted at the discretion of the tryout director.

Please bring the following to the tryout…

• A soccer ball, water bottle and come in soccer gear including shin guards.
• Please complete and provide...

1. IYSA Medical Release Form
2. NISL/US Club Soccer Waiver Form (boys) or IWSL Registration Form(girls)
3. Cheetah Tryout Form
4. Copy of the players birth certificate.
Keep in mind that these are competitive teams and players will be chosen based on attitude, hard work and soccer skills.
You will be contacted after it has been determined whether your child made or did not make the team.
